This means that simply re-muxing the file might have done the job as well, you might want to try that too
I wouldn't be surprised if some people use encoders that have a faulty muxer, that was already the case in the avi days.

Yes that is exactly what I am saying. You don't want to know in how many ways a multiplexer can be broken. OTOH if you use mkvmerge, the result will be guaranteed correct.
