Is this a new disk? If so, it shouldn't have a bad blocks.

Lot of errors on that disk. Send it back and ask a new one?
2 x ultimo4k (dvb-c fbc + 1 dvb-s2, 8.3r/nb) + een et10k/osmini (geen tuners en dus fallbackclient, nb) met een Synology ds214+ (2 x 6 TB) op DSM 7.1.1 in ziggo oost (voormalig @Home) + A1/A2/A3/HB (TechniSat)

From your dmesg output, there is only one partition on the disk? The remainder is just unused space.

What box did you have again?
There are basically two options: an ext4 filesystem, if it can be created with 64-bit pointers, or xfs (which is currently not in the image and needs to be added).

tune2fs -l /dev/sda1
in the output, look for
Filesystem features: has_journal ext_attr resize_inode dir_index filetype needs_recovery extent 64bit flex_bg sparse_super large_file huge_file dir_nlink extra_isize bigalloc metadata_csum
if this feature list includes "64bit", then the linux kernel version of your box supports partitions larger than 16TB.

Not there.
Is there anything of value on this partition, or is it ok to format it again? If it is ok, can you issue this command?
mkfs.ext4 -m0 -C 262144 -N 262144 -T largefile -O 64bit,bigalloc,sparse_super /dev/sda1
and post the output?

Ah, it is still mounted. I thought it didn't mount with all the errors? Or did the repair fix those?
To unmount, issue the command
umount /dev/sda1
and then retry the format.

If you now run
tune2fs -l /dev/sda1
again, is "64bit" now present in the feature list?
